Actor portrayals
What is acquired HO?
Acquired hypothalamic obesity, or acquired HO, isn't like other forms of obesity
It is a unique condition that begins when a brain tumor or injury affects part of the brain called the hypothalamus
It is characterized by weight gain that continues over time and is difficult to manage
People with acquired HO may have other symptoms such as increased hunger, feeling excessively tired, or feeling less active
What makes acquired HO unique?
Weight gain in acquired HO can be difficult to manage because of its unique underlying cause: MC4R pathway impairment in the brain
The MC4R pathway in the hypothalamus regulates hunger, fullness, and metabolic rate (calorie use). When the MC4R pathway is damaged as a result of an injury, it can:
- Disrupt signals that help balance hunger, fullness, and metabolic rate
- Cause long-term weight gain that is resistant to traditional weight management approaches, including diet and exercise
How does acquired HO cause weight gain?
Acquired HO is caused by an injury to the hypothalamus that damages the MC4R pathway
Messages that regulate hunger, fullness, and metabolic rate (calorie use) travel along the MC4R pathway in the brain. An injury to the hypothalamus can permanently disrupt the MC4R pathway.
Damaged MC4R pathway
Messages are interrupted
Messages about using calories and feeling full that travel from the brain to the rest of the body are interrupted

In acquired HO, MC4R pathway impairment causes increased hunger and a lower metabolic rate. This can cause weight gain that can be hard to control on your own.
Targeting MC4R pathway impairment—the source of acquired HO—is an important piece of the long-term management of weight gain and hunger.
